Please note that the Head of the Passport Office of this Consulate will be visiting the Honorary Vice Consulates in Port Elizabeth and East London on the dates indicated below, in order to collect the fingerprints and signature of Italian citizens who need to apply for a new passport:

- send this passport application form to the Honorary Vice Consul. Women should complete the form with their maiden surname.
- If the applicant has minor children OR is a minor, this consent form also has to be completed and will have to be signed by the other parent at the appointment at the Vice Consulate;
Children under 12 years of age do not have to give fingerprints.
